I, the parent/legal guardian of the registered player, acknowledge that participation in Shark City Hoops basketball camps and training involves inherent risks of physical injury. In consideration of my child's participation, I release and hold harmless Shark City Hoops, its coaches, staff, and the host facility from any claims, liability, or expense arising from injury or illness sustained during program activities, except in cases of gross negligence. I confirm my child is physically able to participate and that the medical information provided is accurate. I authorize Shark City Hoops staff to seek emergency medical treatment if needed.
